In checking the box below I agree that 1. CLASS DURATION: All group classes are 50min long 2. EXPIRATION DATE: All group class packages have an expiration date and do not roll over to the next session. Please use your lessons before expiration date. 3. MISSED CLASSES: Missed classes do not roll over to the next session. Please take advantage of our practice labs within your session, class review videos or private lessons to make up for the material missed. 4. INSTRUCTORS: We all work as a team and follow an established syllabus to provide you the best service possible. The studio cannot guarantee the same instructor throughout the duration of your lessons. 5. REFUNDS: We love you, but there are NO REFUNDS on services and items purchased online or in studio. There will be no credit given for the classes registered for but not attended. 6. RELEASE: By signing this contract you agree to release and forever discharge, DF Dance Studio and their employees from any liabilities growing out of or related to any activities in which you may participate in. You understand that this is a full and complete release for all injuries or damages, which you may sustain as a result of your participation in any activities, regardless of the specific cause thereof. 7. CLASS CHARGES: You agree that your credit card on file will be charged for all classes attended. 8. PHOTO RELEASE: Dance Studio will be taking photographs/videotape of you during the course for publicity, promotional and/or educational purposes (including publications, presentations or broadcast via newspaper, internet or other media sources). I hereby grant DF Dance Studio and its respective affiliates and licensees the right to use my name, likeness, portrait, recorded voice and biographical material, in order to advertise, promote or publicize DF Dance studio and it’s affiliates.
